Conditions of Membership
Please carefully read the conditions of membership of Rockdale Library Service.
You must accept these conditions of membership before your membership can be granted.
As an adult member I undertake to observe library rules, make good the loss or damage to any item lent to me, pay any charges or fines incurred and notify the library of any change of address in accordance with NSW Library Act 1939.

As a parent or guardian of the person in whose name this application is made, I undertake to observe library rules, make good the loss or damage to any item lent to me, pay any charges or fines incurred and notify the library of any change of address in accordance with NSW Library Act 1939.
I note that children under the age of 7 must be supervised by a responsible person who is 18 years or older. Supervision requires that the person be within sight of the child at all times.

Whilst using Computer Services provided by Rockdale Library Service I agree to abide by the terms of use as outlined in the Internet Use Policy.
I note that restriction of a child’s access to the Internet is the responsibility of the parent or guardian.

I agree to abide by the Copyright Regulations which apply to published material in accordance with the Act, 1968 when photocopying or printing material from resources provided by Rockdale Library Service.

If at anytime during my membership I no longer wish to receive the library's monthly eNewsletter I will notify the library.

Rockdale Library Service agrees to abide by the Australian Privacy Laws which regulate the collection, use and disclosure of personal information about individuals.
